These two words get used interchangeably in conversation and on packaging, and the result is that a lot of men buy the wrong one and then conclude that nothing works. They are not competing versions of the same product. They solve two different problems, and knowing which problem you have takes about ten seconds.

If a product does not list an aluminum compound among its active ingredients, it is a deodorant, whatever the front of the package suggests.
The regulatory line is the useful tell. In the US an antiperspirant is classified as an over-the-counter drug because it changes a bodily function, which is why the back of the package has a Drug Facts panel listing an active ingredient and a percentage. A deodorant is a cosmetic and does not. Flip the bottle over. If there is no Drug Facts panel and no aluminum compound, it will not keep you dry.
Why Sweat Is Not What Smells
This is the part that reframes the whole decision. Fresh sweat is close to odorless. You have two kinds of sweat glands: eccrine glands across most of your body producing thin, watery sweat for cooling, and apocrine glands concentrated in the underarms and groin producing a thicker, protein and lipid-rich secretion. Skin bacteria feed on that second type, and their byproducts are what you actually smell.
So an antiperspirant reduces odor indirectly by removing the moisture the bacteria need, while a deodorant attacks the bacteria and the smell directly without touching sweat volume. Two mechanisms, two different failure modes.

The American Cancer Society and the National Cancer Institute have both stated that there is no conclusive evidence connecting antiperspirant use to breast cancer or Alzheimer's disease. Choosing to avoid aluminum is a perfectly reasonable preference. It is just worth making that choice knowing the evidence rather than assuming a risk that has not been established.
What Happens When You Switch to Aluminum-Free
Expect two to four weeks of it feeling worse. You are not detoxing, and there is no aluminum stored in your armpits working its way out, despite how that transition is often described. What is actually happening is simpler: the plugs in your sweat ducts clear, output returns to its natural level, and you notice it because you have not experienced your own baseline in years. Your skin bacteria population also rebalances during this window, which is the part that affects smell.
It does settle. But if you switch and then judge the product on week one, you will conclude it failed when what you were really measuring was the absence of the antiperspirant you stopped using.
A Reasonable Default
If you do not want to think about any of this: buy a combined antiperspirant deodorant, apply it to fully dry skin at night rather than in the morning, and keep underarm hair trimmed short.
